My Aged Care is the Australian Government's starting point on your aged care journey to find and access the government-funded services you need.
Applying for an Assessment
The first step to access government-funded aged care services is to check your eligibility for a face-to-face assessment. You can call My Aged Care on 1800 200 422 or apply online https://www.myagedcare.gov.au/assessment/apply-online. Please note: If you have previously registered with My Aged Care, you cannot use the online form and will need to call My Aged Care.
For a face-to-face assessment you should:
- have your Medicare card and one other form of ID proof - such as DVA card, driver's license, healthcare card, or passport
- have a copy of any referrals from your doctor
- consider if you would like a support person present
- have any information you already have about aged care services that you may want to discuss
- have contact details for your GP or other health professionals
- consider if you need special assistance to communicate, such as a translator or Auslan interpreter
- have information on any support you receive
The assessor will have a copy of the information you gave to the My Aged Care contact centre. This give them an idea of what support you might need and they will also ask about:
- what support you already have, and if it will continue
- you health, lifestyle and any health concerns
- how you are going with completing daily tasks and activities around the home
- if you have problems with your memory
- any issues relating to home and personal safety
- family and community activities
- speaking to your GP or other health professionals
Holy Family Services assesses individual needs and uses the information in your support plan to provide the appropriate care and accommodation.

We encourage you to make an appointment to view our aged care facilities in Marayong, Western Sydney, speak to residents and meet the staff to ask questions and discuss your relative’s care needs.
We recommend you look out for the following things when considering an aged care provider:
- A comfortable environment, surroundings suitable and acceptable by your relative
- Staff qualifications, government certifications and awards
- A multi-disciplinary team of health professionals to ensure an appropriate level of care is available
- What levels of aged care services are provided and whether your relative can be accommodated if their care needs change
- A menu that is healthy and varied
